ABOUT THE AWARDS

The AIDS Legal Council of Chicago and Windy City Times are inviting members of the public to submit nominations for the 2012 Advocates of the Year Awards. Established in 1994, the Awards recognize individuals who, through advocacy, philanthropy, education, medical care, volunteer work and direct client services, are fighting the epidemic and helping to improve the quality of life of people with HIV. The 2012 Selection Committee welcomes nominations for individuals or organizations demonstrating outstanding achievement in HIV care and advocacy, with special attention to those who are distinguishing themselves in the following areas:

Advocacy

The 2012 Selection Committee is interested in recognizing the work of community activists, elected officials, religious or civic leaders, advocacy groups or other outstanding candidates who demonstrate

exemplary leadership in advocating for the rights and well-being of people with HIV.

Health Care

The 2012 Selection Committee strongly encourages nominations for professionals who demonstrate excellence in providing health care to people with HIV. Candidates may include physicians, nurses, dentists, mental health providers, pharmacists or other health professionals who provide outstanding care to their patients with HIV.

Support Services

The 2012 Selection Committee is interested in paying tribute to individuals who provide direct, non-medical services to people with HIV. Ideal candidates may include case managers, counselors, housing advocates, legal professionals, social workers, and others who deliver outstanding support services and care to their clients.

Philanthropic & Volunteer support

The Committee is interested in recognizing individuals, foundations, firms, businesses and others who lend their time, expertise and financial support to programs for people with HIV. Donors and funders, special volunteers, board members, and individuals who render pro bono services are ideal candidates.

GUIDELINES

Education & Prevention

The Committee welcomes nominations for individuals dedicated to training and educating consumers, medical providers and members of the public about HIV treatment, adherence, research and prevention. Ideal nominees include prevention specialists, peer educators and others providing vital information about HIV and AIDS.

Emerging leadership

The 2012 Selection Committee is interested in acknowledging individuals in any of the above areas who are demonstrating remarkable promise as advocates for people with HIV. If your candidate is under the age of 35 or has less than 5 years of experience in the area of HIV service or advocacy, please check the appropriate box on the nomination form (below).

Nominating

The Advocates of the Year Award Selection Committee receives many outstanding nominations every year.

When preparing your nomination, keep the following in mind:

-Do not assume that members of the selection committee will be familiar with your nominee. Provide as

many concrete examples of the candidate’s achievements as you can and take special care to be accurate

with facts, names, dates and any other relevant information.

-Please make sure to submit a completed nomination form, a letter describing why your candidate would

make a worthy recipient of the award (2-page max.), and two additional letters of support, not to exceed

two pages each. If you are submitting supplemental information (such as newspaper clippings, resumes or

articles), please keep to 5 pages or less.

– To ensure the most diverse pool of candidates possible, you are encouraged to submit applications for

people of color, young emerging leaders and individuals who are public about their HIV status.

– The Committee will generally favor applications for nominees who have gone above and beyond their

routine professional duties in providing services and advocating for persons with HIV.

– Submit all materials by Friday, December 9, 2011 via mail, email, fax or delivery to:

Ricardo Cifuentes

Director of Development

AIDS Legal Council of Chicago
