Center on Halsted, in compliance with the City of Chicago public health order effective Jan. 3, requires proof of COVID-19 vaccination for anyone (ages 5 and older) gathering in its ground-floor lobby area.
Other events and programs, such as Open Gym, may also require proof of vaccination. Per CDC safety guidelines, the Center, 3656 N. Halsted St., requires masks be worn over the mouth and nose of employees and visitors regardless of vaccination status.
Acceptable proof of vaccination includes:
Vaccination card
Photocopy of vaccination card
Digital record or app, or
Printed record from vaccine provider
The Center urges everyone to follow risk-reduction practices such as:
Maintaining appropriate social distancing of at least six feet whenever possible.
Avoiding touching the face, especially the eyes, nose and mouth.
Covering the mouth and nose with a tissue when coughing or sneezing, then throwing the tissue in the trash and wash your hands.
Regularly and thoroughly washing hands with soap and water for at least 20 seconds.
Carefully monitoring one's health, including for symptoms of COVID-19 (i.e., fever, cough, chills, respiratory issues, loss of taste or smell)
Staying home if not feeling well or have a temperature above 100.4 (The Center has a temperature kiosk on the second floor of the building, next to the elevator bay.)
In addition, the Center on Halsted provides masks, gloves and hand sanitizer to all in need.
Also, the Center is offering free COVID PCR and rapid testing on site on Monday-Wed., Friday and Saturday, 12-6 p.m. No appointment or insurance is necessary.
The area is open every day 8 a.m.-9 p.m.
