Heartland Health Outreach will hold its annual holiday brunch—commemorating the 25th anniversary of Vital Bridges Center on Chronic Care—on Sunday, Dec. 15, at the Four Seasons Hotel, 120 E. Delaware Pl., 11:30 a.m.-3 p.m.
Guests will partake of entertainment, Four Seasons’ cuisine, live and silent auctions, and a cash raffle.
